The Ohio Department of Health has created a link with COVID-19 Vaccine Provider Locations within Ohio.
The site lists the vaccine schedule along with Provider locations. The Provider locations are searchable by name, county or zip code. Once an area has been selected the providers are shown, after determining the provider click on the provider web site and follow the directions to schedule an appointment. As new providers are registered they will be added to the chart.
